Clinical Case Manager Part-Time Weekends Only 2 ten hour days - open to flexibility Summary: Provides Utilization Review and Case Management functions for the organization. Performs preadmission, concurrent and retrospective review for all payers. Completes assessments for the purpose of identifying discharge-planning needs. Provides discharge-planning services for select patient populations. Collects data to support performance improvement initiatives. You will be working with a tenured case management team who have excellent education and experience. In addition, you will have the opportunity to work other shifts.
