Standard Qualifications: Henrico Doctors Hospital, the Parham Campus, is a 200-bed acute care facility located in Richmond, VA. As part of our full range of healthcare services, we specialize in heart and stroke care, women's health-including high-risk pregnancies, maternity and neonatal intensive care, orthopedics, groundbreaking diagnostic imaging, and inpatient rehabilitation. The inpatient rehabilitation unit is a 36 bed unit located within the hospital on the campus of Henrico Doctors, Parham Campus. We specialize in a wide range of health conditions including stroke, traumatic brain injury, neurological disease, amputation, orthopedics, pulmonary and cardiac disease, and debility from complex medical conditions. Our goal is to provide the highest quality rehabilitation services to each and every patient through delivery of services provided by a highly skilled multidisciplinary team including Physical, Occupational, Speech and Language Therapists, Nursing, Clinical Rehab Specialists, Admissions Coordination, PPS coordination, Case Management, and Medical Director. Position Responsibilities: The Case Manager/Social Worker will help facilitate discharge needs of patients with emphasis on culture, financial considerations and family dynamics. Department Specific Criteria: 1. Assists patient/families with psycho-social, cultural and financial considerations that may impede recovery. 2. Formulates plan for post-hospital care. 3. Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to the ages of patients. 4. Facilitate weekly team meetings and family conferences as needed. 5. Completes psycho-social assessment upon admission. 6. Performs insurance updates, as requested.
