Our board-certified physicians direct complete primary care, specialty care and urgent care services for those who cannot leave home to get to doctor's visits. At Kindred House Calls, our house call physicians, nurse practitioners, physician assistants and care team deliver compassionate, high-quality care to patients in their homes or places of residence. Our providers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families. Our office staff supports our providers and our patients in real time on a daily basis to help to improve overall patient satisfaction. As a Nurse Practitioner, you will: Be responsible for providing an advanced level of nursing practice to patients and families, servings as a clinical resource for staff and interdisciplinary team. Essential Functions: Provides independent primary care, consultation and advice to others as a highly specialized practitioner. Makes patient visits, assumes on-call duties, exercise prescriptive authority and directs Interdisciplinary Team Meetings. Makes home visits to all new patients within 1 week of admission and monthly thereafter. Makes visits to patients who have complicated diseases, where re-certification is in question, or as requested. Completes complicated admissions; i.e., hospital to homecare, ventilator withdrawal, complex ethical situations. Fulfills CMS Face to Face requirements. Keeps and maintains appropriate records relating to all professional services rendered. Provides appropriate internal and/or external clinical education. Attends team meetings, nursing meetings and staff meetings regularly Assists in the development of patient care policies and procedures. Serves as a clinical resource to clinical staff. Participates in the education and mentoring of the nursing staff to promote quality of patient care. Participates as requested in quality assurance activities. Adheres to practices of confidentiality regarding patients, families, staff and Kindred practices. Performs other related duties as assigned or requested. Masters of Science in Nursing from an accredited school of nursing Current Nurse Practitioner certification/license. Current Driver’s License. Must have reliable transportation and insurance. Minimum of five years Nurse Practitioner experience. Nurse Practitioner specialty in Geriatrics, In-Home Care or Hospice preferred. Excellent leadership, interpersonal, written and verbal communication skills. Independent decision making and self-motivation. Working knowledge or understanding of palliative care techniques, Medicare regulations, home health regulations, and interdisciplinary process.
