SUMMARY STATEMENT: The incumbent shall be responsible for providing case coordination and a full range of social work services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary fashion. ESSENTIAL DUTIES: Participate in continuing education/professional development activities. Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others. Adhere to policies and practices of applicable professional organizations, client hospital and RehabCare Group. Assist with the development of student programs as requested. Contribute to client relations and marketing activities. Maintain competency, licenses/ certifications, and memberships in applicable professional organizations. CLINICAL AREA: Participate in pre-admission screening as requested. Conduct initial assessments. Develop individualized treatment plans. Provide age appropriate therapeutic interventions. Evaluate patient and family response to treatment. Document the course of patient care including progress made and continuing need for treatment. Establishes and maintains a therapeutic relationship with patients and families. Provide patient and family education on tasks and resources that will assist with continued functional independence. Obtain pre-certification and re-certification of patient's insurance coverage as appropriate according to policy and procedure. INTERDISCIPLINARY TEAM AREA: Coordinate treatment activity with other disciplines and team members. Participate in team business meetings. Function as a resource for and provide team members with inservice/cross-training on social work knowledge and skill areas. Contribute to development of improved efficiency and productivity of unit clinical/administrative functions. QUALITY MANAGEMENT AREA: Participate in unit quality management activities. Collect quality review data. Participate in survey preparation activities. Interact with assigned program consultant. Participate in unit safety committee activities. Contribute to the delivery of cost effective care. Qualifications: If you are a current Kindred/RehabCare employee Click Here . QUALIFICATIONS: The incumbent shall, at a minimum, be a graduate of a CSWE approved masters degree program and hold applicable and current license/certification to practice. ACSW certification is desirable. Education/experience in working with an adult and geriatric population is necessary; education/experience serving people with physical disabilities is desirable.
