Facility: This is a unique opportunity to provide care to elderly patients aged 65+. Applicants must be comfortable providing care to patients in their homes.

Day-to-Day: NPs will be providing in-home health assessments (blood pressure checks, medication overview, and providing referrals for specialty care). EKG may be required and equipment will be provided. Charting must be completed the same day.

Schedule: Applicants must be willing to commit to working 4-5 days/week, Mon-Fri or Mon-Thurs, covering 16 weeks.

There is an opportunity to extend with this job for an additional 16 weeks following a mandatory 2 weeks off.
