Description
The Behavioral Health Therapist, LCSW is primarily responsible for providing general mental health and substance abuse counseling services as a member of an interdisciplinary team. This licensed individual is responsible for assessment, treatment planning, and implementation of an approved plan of patient care which is provided through individual and group sessions. The Behavioral Health Therapist works on a dynamic interdisciplinary team and seeks opportunities to collaborate, deliver best practices, and deliver whole health opportunities for wellness.Responsibilities: Conduct member assessments and establish therapeutic treatment goals Work cooperatively with Interdisciplinary Care Team for best outcomes Facilitate individual and group counseling; maintain thorough and confidential records of observations and treatment within an electronic health record Evaluate progress and effectiveness of counseling interventions Coordinate and integrate care with primary care and pain management providers for members receiving prescribed medications Systematically and continually assess the member's coping mechanisms and progress Actively coordinate care with the multi-disciplinary team to ensure effective and coordinated care from the initiation of treatment to the time of discharge Evaluate and communicate member's discharge planning needs and readiness for discharge Provide counseling services and work closely with members to identify behaviors or situations that interfere with their recovery Refer members to community programs and services; recognize and utilize behavioral health teaching opportunities and available resources or materials Document all activities in accordance with policy and regulatory requirements Engage member and family in a culturally competent manner Meet regularly with clinic providers and with clinic leadership Receive regular clinical oversight as required for best practice and skill development Required Knowledge, Education & Experience: Bachelors degree in Social Work, Counseling, or a directly related field of study Minimum 2 years of experience working in a behavioral health setting; experience should include substance abuse and addiction therapy Current and unrestricted Licensed Clinical Social Worker (LCSW) from the Illinois State Board of Behavioral Health Examiners Able to receive and maintain CPR/BLS certification within 60 days of hire Demonstrated knowledge of medications and their indications, common side effects, reactions, and interactions of medications prescribed Working knowledge of the current version of the Diagnostic and Statistical Manual of Mental Disorders (DSM) and the International Classification of Diseases (ICD) Proficiency with Microsoft Office applications, EMR systems (e.g., Athena), and web-based technologies Experience with and comfort using keyboards, laptops, tablets, and other electronic and portable devices for documentation

Highly Preferred Skills, Abilities & Qualifications:Bilingual; able to read, write, and speak Spanish and English proficiently a plus Experience working in an integrated or complex care model with primary care and pain management Experience in working with diverse and underserved populations with a strong understanding of multicultural issues Demonstrated knowledge and experience with crisis intervention techniques
