As a Certified Nursing Assistant for our Hospice Division, you will: Provide personal care and selected housekeeping and therapy services to ill, disabled, or injured patients within their home Assist with basic activities and daily living needs of patients Perform work assignments in accordance with organization’s established policies, procedures, and protocols as directed by your supervisor. Meet with your PCC, on a regularly scheduled basis to assist in identifying areas for improvement. Identify patients appropriately before administering treatments, serving meals, etc., as necessary. Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner. Communicate changes in the patient’s condition to the Team Leader/PCC as soon as practical. Report all accidents and incidents you observe on the day that they occur to Team Leader/PCC. Qualifications Current and active C.N.A. license Minimum of six months of experience as a Certified Nursing Assistant preferred Hospice experience a plus Ability to provide partial or complete living assistance to patients without assistance from other healthcare workers Good organizational and communication skills
