Must be graduate of an accredited MD or DO program with fellowship in Pain Management. Board certification or eligibility in both Pain Medicine and Addiction Medicine. Must have completed an ACGME accredited fellowship. Must be Board Eligible or Certified by The American Board of Physical Medicine and Rehabilitation or The American Board of Anesthesiology. Must have or be able to obtain a Florida medical license and DEA. 2-5 Years of experience in interventional pain medicine preferred. Physical Medicine and Rehabilitation preferred, but not required.
