• Dates Needed: ASAP


• Reason for Need: Medical Leave of Absence (MLOA) absence


• Hours: 7:00 AM to 3:00 PM / Call hours: 3:00 PM to 7:00 AM


• Schedule: No Rotation


• Call Scheduled: CRNA call is OB only


• Call Response Time: 30 minutes


• Type of Cases Required: ENDO, OB/GYN, Peds, Urology, ENT, General, Oral, Ortho, Vascular, Trauma, Ophthalmology


• Proficiency Required: Spinals, Blocks, Epidurals


• Credentialing Timeframe: 60-90 days (Temporary opportunities available)


• Caselogs Required: Proof of epidurals, spinals, and blocks


• Number of Providers During the Day: CRNA 3:1, MD 2:2


• Number of OR’s: 4


• Number of Endo Rooms: 1


• EMR System: Epic


• Vaccination Required?: Need to be vaccinated / Accepting religious / Medical exemptions


• Licensure Requirements: ACLS, BLS, PALS
