Position Details:

  • On-site presence required; no remote option
  
  
  • 24-hour call coverage with beeper call
  
  
  • Locums will NOT be responsible for supervising other providers
  
  
  
  


• EMR: EPIC


• Trauma Level: Level II


• Travel Reimbursement: Client will reimburse for travel

Credentialing Requirements:

To expedite the credentialing process, please ensure the following items are readily available for your provider at the time of acceptance:

• MD diploma


• Internship/residency/fellowship certificates


• All medical licenses


• DEA


• Board Certification


• Life support cards (BLS)


• Current and previous certificates of insurance


• Driver’s license


• Professional color photo/headshot


• CMEs for the past 2 years


• Immunization records, including TB, Flu, HepB, Tdap, Varicella, and COVID


• References


• Case logs

Submission Requirements:

• Board Certified - Required


• Oregon license - Required


• DEA and CSR - Required


• BLS - Required


• Local candidates - Highly preferred

COVID-19 Vaccination:

• Proof of COVID vaccine is required at the time of submission


• Must be submitted for consideration to work at this facility
