Mileage Reimbursement: Only for local providers

Malpractice Rates: MUST be included in the rate

Billing Method: Reverse billing; provider clocks in/out using Kronos

Submission Requirements

MI License: REQUIRED, no exceptions

NBCRNA Certification: Required (new grads welcome)

BLS and ACLS: Required

Background: Clean background, no malpractice for quick credentialing

Local Providers: Drive-in providers required, no exceptions

Rates and Availability: List rates and specific availability at the time of name clear
