Collaborates with 43 skilled nursing facilities and many assistive living/adult homes across our service area.

Physicians meet routinely with the entire hospice interdisciplinary group (nurses, social workers, chaplains, volunteers) for care planning to meet each patient’s and family’s specific needs, serves as a resource to and collaborates with the hospice patient’s community medical providers (including prescribing medications), is responsible for certifying the hospice patient’s initial and ongoing eligibility for hospice services, helps to determine the appropriate level of hospice care, and may serve as the hospice patient’s attending physician. Physicians are supported by a group of excellent nurse practitioners who work collaboratively with the interdisciplinary team on symptom management and determining hospice eligibility.

Physicians visit patients in their homes, nursing homes, and hospitals as necessary, and routinely participates in the education of medical students, NP and PA students, as well as residents and Geriatrics Fellows. On-call responsibilities will be shared by a group of 5 hospice physicians.

Fellowship in Geriatrics or Hospice & Palliative Care is a plus but not required.
