Description
Geriatric Nurse Practitioner needed in Montana to provide care to nursing home residents who may not have a physician or whose physician may not follow them in a long-term care setting. The Extended Care Center and Transitional Care Center treat residents in the community and across Montana. The facility serves about 146 residents. A transitional care program focuses on returning the resident back to the community. The remainder lives in a residential care setting that is licensed as a Skilled Nursing Facility. Employer is currently in the midst of developing several other senior services such as Assisted Living, Independent living and an All Inclusive Care Program for the Elderly. Presently there is one Internal Medicine Physician and a FNP who see patients in the Community and a Gerontologist based at the Nursing homes. They see 20-25 residents/daily. Salary between $87-95K with significant upside potential due to production.
Skills
Minimum one year geriatric experience. Current Montana license preferred.
