• Must be BC/BE in Internal Medicine, Family Medicine or Geriatrics
• Exclusive focus on senior adults with complex, chronic conditions
• Small patient panel size: 400 annually (see patients an average of once per month)
• Mon. – Fri., 7:30 am – 5 pm
• Practice call – phone call only
