• Completed Masters Degree Level Nurse Practitioner Program or Physician Assistant Program from an accredited college/university
  
• Active unrestricted applicable licensure
  
• CPR Certification
  
• NPI & DEA
  
• Current National Board Certification
  
• BLS-Certification
  
• DOT Certified
  
• Prefer OCC Health Experience
  
• Ideal candidate will have at least three (3) years primary care practice experience, the ability to work independently, and experience working with and Electronic Medical Record.  
  
• Provider should be comfortable seeing patients ages 2 and up
