Momentum Healthcare Staffing is seeking a Nurse Practitioner for a permanent Palliative Care position in Findlay, Ohio. This role provides the opportunity to deliver compassionate care to patients with serious or chronic illnesses, collaborating closely with physicians, nurses, and multidisciplinary care teams.

Key Responsibilities:
Assess, diagnose, and manage patients requiring palliative care
Develop individualized care plans focused on symptom management, quality of life, and patient-centered goals
Provide patient and family education regarding disease processes, treatment options, and end-of-life care
Collaborate with physicians, nurses, and support staff to optimize patient care
Maintain accurate and timely documentation of care plans and patient interactions

Qualifications:
Current Nurse Practitioner license in Ohio (or ability to obtain)
Certification in Palliative Care or relevant clinical experience preferred
Strong clinical judgment, communication, and patient advocacy skills
Experience in palliative, hospice, or chronic care management preferred
