Description
Experienced Physician Assistant sought for primary care position in a large multi-specialty, integrated practice health center. Experience with complex disabled or group home population a plus, but not required. Competitive salary and generous benefit package, CME and additional differentials for convenience hours available. Health Center is PCMH III certified with partnerships in State-wide ACO and local Safety Net Provider Organization. Responsibilities include full scope of medical care (ages 18 and up to include):
• Routine health maintenance
• Health promotion
• Disease prevention
• Chronic Disease management
• Clinical Outreach
