Position Highlights
•  Secure full-time employment with an expanding PACE program
•  Leadership position with direct influence on clinical operations and program growth
•  Balanced schedule includes 1–2 clinical days per week
•  Leadership role shaping provider education, quality improvement, and operations
•  Collaborate with an established team with two APPs and a multidisciplinary support staff
•  Guide PACE specific training for newer clinicians and mentor seasoned providers
•  Evaluate and improve care delivery using P&L and ROI analysis to drive performance
•  Build systems to ensure clinical excellence, utilization efficiency, and patient satisfaction

Credentials
•  Experienced, boarded MD/DO with a passion for Geriatrics
•  Family Medicine or Internal Medicine training
•  Experience in PACE, FQHC, or other value-based care models strongly preferred
•  Demonstrated success in provider education and operational leadership
•  Collaborative, mission driven approach with strong stewardship and communication skills
