Work Dates Needed: March 23 - July 17th
Preferred Schedule: M-F 8a-5p
Worksite Setting: Outpatient
Scope of Work: 50/50 kids/adults
Licenses, Certifications, Requirements: 3 years DDS FQHC experience a must. Active CA license, BC, BLS, active DEA. The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
