In this role, you will move away from the high-volume grind and focus on building genuine, long-term relationships with your patients. Working alongside a dedicated physician, you will manage non-complex cases, prescribe necessary medications, and provide holistic care that aligns with Medicare Advantage goals.

Skills

To ensure success in this Value-Based Care environment, the following qualifications are sought:
» Several years of dedicated Primary Care experience, with a specific focus on Geriatrics.
» Previous experience working within an Integrative Medicine, Family Practice, or Pain Management setting.
» Prior exposure to Value-Based Care models is highly preferred.
» Proficiency with eClinical Works (eCW) EMR system is a plus.
» Active BLS or ACLS Certification is required.
» Must hold a Family Nurse Practitioner license in the state of practice or have one currently in process.
