Medix is partnering with an expanding healthcare team in New Hampshire to welcome a dedicated Nurse Practitioner to a stable, facility-based role. In this position, you will join an interdisciplinary team providing high-quality primary and preventative care specifically to residents within a skilled nursing and long-term care environment. Because this opening is due to the acquisition of a new facility, you will have the unique opportunity to help shape the care culture for this patient population from the ground up.This role is designed for clinicians who value stability and work-life balance. You will be based at one stable location—no floating between facilities—and enjoy a flexible 4-day work week with absolutely no on-call requirements. Your daily focus will include managing acute changes in condition, conducting advanced care planning, and collaborating closely with nursing staff and primary care physicians to ensure the highest well-being for the elderly residents under your care. Location: New Hampshire Facility Type: Skilled Nursing Facility (SNF) Start Date: Immediate / Summer 2026 Length of Assignment: Permanent / Full-Time Schedule: Monday – Friday | 8:00 AM – 5:00 PM (4-Day Work Week) State License Needed: New Hampshire (Active/Unrestricted)
Skills
To ensure the highest standard of care for residents with complex chronic conditions, candidates should possess: » A current, unrestricted Nurse Practitioner license in the state of New Hampshire. » Active Board Certification and DEA licensure. » A strong clinical background in Geriatric, Adult, or Family specialties. » Prior experience working within a long-term care or skilled nursing facility (SNF) setting. » Proficiency in managing electronic medical records (EMR) and documenting complex care plans. » The ability to lead advanced care planning and manage patients with multiple chronic illnesses independently. » Excellent collaborative skills for working within a multidisciplinary medical team.
