Rates: $110/hr

Opportunity Highlights

Clinical Role & Responsibilities
This is an assessment-only role. You will focus on identifying clinical needs and chronic conditions to ensure patients receive proper follow-up from their PCPs.
Daily Volume: Conduct 5–6 comprehensive assessments per day.
Focus Areas: Perform annual risk assessments, chronic condition reviews, and social determinant screenings.
Diagnostics: Perform A1C testing when applicable; no other invasive screenings are required.
Care Coordination: Identify and escalate urgent findings directly to the patient’s Primary Care Provider.
Prescribing: None. This role does not require prescribing or active treatment plans.

Provider Requirements
Board Certification: BC Family Nurse Practitioner (FNP) (Required).
Licensure: Active South Carolina (SC) Medical License (Required).
Medicaid Enrollment: Must be currently enrolled/credentialed in Medicaid. Please confirm status at the time of submission.
Logistics: Must be comfortable with both telehealth and traveling within Richland County for home visits.
