### About the Opportunity\nMedix is seeking a Nurse Practitioner for a facility-based team. This role offers a unique flexible schedule, allowing the provider to work a 4-day work week in a long-term care setting.\n\nLocation: Ohio\nFacility Type: Skilled Nursing Facility\nStart Date: January 5, 2026\nLength of Assignment: Direct Hire\nSchedule: Mon - Friday (8am - 5pm EST) 4 days/week (flexible selection)\nState License Needed: Ohio\nPay: Personalized compensation packages, including competitive rates and applicable stipends, will be discussed in detail with your recruiter.\n\n### Responsibilities\n--> Provide primary and preventative care for long term care patients.\n--> Manage Residents with acute changes in condition.\n--> Communicate with nursing home staff regarding care plan changes.\n--> Collaborate with primary care physicians.\n\n### Required Skills & Qualifications\n--> Current, unrestricted NP license in Ohio.\n--> Board Certification and DEA licensure.\n--> Clinical background in adult, family, or geriatric specialties.\n--> Experience working in a long-term care setting.
