### About the Opportunity\nMedix is partnering with a growing organization to find a Nurse Practitioner for a Value Based Care Model serving the senior population. The goal is for providers to build deep relationships with patients through longer appointment times.\n\nLocation: Arizona\nFacility Type: Primary Care / Geriatrics\nStart Date: April 20, 2026\nLength of Assignment: Direct Hire\nSchedule: Mon-Fri 8am-5pm\nState License Needed: Arizona\nPay: Personalized compensation packages, including competitive rates and applicable stipends, will be discussed in detail with your recruiter.\n\n### Responsibilities\n--> Serve the senior population in a Value Based Care Model.\n--> Prescribe medicine and work through non-complex cases alongside a doctor.\n--> Conduct 45 minute patient visits.\n--> Participate in 8am team huddles and end-of-day admin.\n\n### Required Skills & Qualifications\n--> Family Nurse Practitioner License in Arizona.\n--> BLS or ACLS Certification required.\n--> Several years of Primary Care experience (including Geriatrics) OR VBC experience.\n--> Experience with eClinical Works preferred.
