We are seeking a Physical Therapist to join a 79-bed skilled nursing and rehabilitation facility serving post-acute rehab and long-term care residents. This is a great opportunity to work in a busy rehab setting with a diverse caseload and strong demand for therapy services.

Responsibilities:
Evaluate residents and develop individualized treatment plans
Provide mobility training, therapeutic exercise, and rehab interventions
Document care in EMR and maintain facility compliance
Collaborate with the interdisciplinary care team

Requirements:
DPT or Master’s Degree in Physical Therapy
Active state PT license
BLS certification
Skilled nursing or rehab experience preferred
Rural or resource-limited healthcare experience is a plus
