Details/Requirements:
--Monday-Friday, 40-45 hour workweek. 6:30 arrival and exits are 1pm, 3pm, and 6pm
--No Call
--Supervision, MDs will do blocks
--Cases: Endoscopy (35%), Urology (25%), Ortho/spine/podiatry (25%), gyn/general, eyes, ENT (5%)
--Indiana license needed
--EMR: Paper
