Position Overview

CRNA coverage for a hospital-based anesthesia service in Rochester, Indiana.

Independent / CRNA-only practice (no supervision).

Typical daily coverage includes 3 providers.

Case Mix / Skills Needed

Mix of OR and OB cases

ENDO, OB/GYN, ENT, General, Ortho, some Peds

Must be proficient in spinals, blocks, and epidurals

Case logs required (proof of epidurals, spinals, and blocks within the past 2 years)

EMR

Epic

Credentialing / Onboarding

Estimated credentialing timeframe: 61-90 days

Indiana license required; IN PCF required

Certifications: BLS, ACLS, PALS, NRP (AHA only - no exceptions)

Credentialing Items (must be provided)

Education diploma(s) (MSN/CRNA, etc.)

NBCRNA board certificate

Current ACLS and PALS

Case logs (within the past 2 years)

Driver's license (color copy)

Professional photo

Current CMEs

Immunizations: Hep B, MMR, TDAP, TB (within 12 months), Flu (within 12 months), Varicella
