Requirements:

• OH License (preferred)

• BC in EM or FM (if FM, must have relevant EM experience)

• Case Logs (Last 12 months/2,000 hrs)

• Procedures logs

• Proof of COVID Vaccination
• DEA (must obtain prior to start date)
