Position Highlights
• Work with a team of nurse case managers, behavioral health professionals, and medical assistants to treat patients with multiple chronic conditions.
• Family Medicine/Internal Medicine outpatient clinic seeing pts age 16 and up with multiple chronic conditions.
• Physicians with 2 yrs outpatient experience including chronic disease management.
• 8 to 5 Mon - Fri with no call.
• 10 to 15 patients per day to full-time.
