Compensation & Perks - Base Salary: $130K+ (flexible based on experience)
- Bonus: Up to $10K annually

Schedule = Work-Life Balance - MondayFriday, 8:00 AM 5:00 PM
- No nights. No weekends.
- Limited call (only 34 weeks per year)
- Call is triaged youre rarely contacted

Your Care Team Includes: - MA
- Scheduler
- Referral Specialist
- Social Worker | Pharmacist
- Access to Endocrinology & Podiatry support
As a Nurse Practitioner, youll deliver comprehensive primary care in a collaborative, team-based setting. Youll manage chronic conditions, guide patients through their care journey, and build meaningful long-term relationships.
Responsibilities include: - Patient assessments, exams & diagnostics
- Chronic disease management (diabetes, COPD, CHF, dementia, etc.)
- Developing personalized care plans
- Patient & family education
- Coordinating care with interdisciplinary team
- Clinic-based care with some home visits & telehealth
What Were Looking For - Board-Certified NP (Adult, Adult-Gero, or Family)
- Active NP license & DEA (or eligibility)
- 2+ years of outpatient primary care experience required
- Comfortable working independently
- Experience with value-based care (preferred)
- Athena EMR
- chronic disease management, wound care, injections, ABIs, or imaging interpretation
