Job Description:

• Responsible in partnership with the Clinical Team Manager, for ensuring that the practice team functions effectively as a unit


• Manage and develop the care team (NP, Physician, Behavioral Health Specialist) understanding both their current performance and long-term professional goals


• Serve as a visible team leader; solve problems, make sound decisions; help others with sound decision making, empower teams to work at and achieve their best


• Ensure the practice adheres to evidence-based practice and engage patients in care


• State of the art practice setting designed for a team-based practice model


• The practice offers smaller panel sizes and the opportunity to lead while working with a true team


• Lead systemic change by building relationships with outside medical consultants to market the practice model to new patients and stakeholders


• Board Certified in Internal, Family or Geriatric Medicine


• Currently licensed or ability to obtain licensure in the state of Colorado
