Primary care physician with HIV experience needed in Baltimore Maryland. Seeking a mission-driven provider to put the principles of team-based care and population health into action for medically complex and socially vulnerable patients Job ID #55046
Job Details
- BC/BE Family Medicine or Internal Medicine
- HIV experienced required
- 13-15 patients per day
- complex care, seeing patients 16 and up
- Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
- provide in-person and telehealth primary/HIV care
- Integrated care team including physicians, nurse care managers, social workers, behavioral health specialists, pharmacists, and community outreach workers

- In house lab, x-ray, IV infusion, ambulatory, ICU and pharmacy
