We are seeking a Case Manager for our facility to ensure
that we continue to provide all patients with high quality, efficient care. We

• You will facilitate the
     interdisciplinary plan of care with a focus on evaluating the
     appropriateness of clinical care, medical necessity, admission status,
     level of care and resource management. 
• You will coordinate activities that
     promote quality outcomes and patient throughput while supporting a balance
     of optimal care and appropriate resource utilization.
• You will identify potential barriers
     to patient throughput and quality outcomes.
• You will facilitate appropriate
     discharge plans. 

Cartersville Medical Center is a 119 bed acute
care hospital, Level III Trauma, Chest Pain Center, Certified Primary Stroke
Center and Gold Seal Inpatient Diabetes program accredited by the Joint
Commission.  The Breast Imaging Center of Excellence at The Women’s Center
provides mammography services that are gold seal accredited by the American
College of Radiology.  The Hope Center, a regional, Commission on Cancer
approved cancer treatment facility, is conveniently located on the hospital
campus and provides over 4,000 treatments with over 300 new patients per year
from a ten county region.

• Preferred case management experience and/or three (2) years of med/surg experience preferred.
• Outstanding customer service abilities and effective listening skills and ability to establish and maintain collaborative and effective working relationships.
• Graduate of accredited School of Nursing with a current GA RN license. 
• Certification in Case Management or Utilization Review preferred.
