Full-time Hospice & Palliative Care Nurse Practitioner needed at a top facility in Vermont!

Currently looking to hire a dynamic and compassionate Nurse Practitioner with a passion for end of life care. Primarily responsible for providing comprehensive hospice and palliative care in consultation with the Hospice Medical Director. In collaboration with the interdisciplinary team, patient and family, the Nurse Practitioner provides consultation, assessments, and evaluation of the plan of care in the hospital, medical office and home settings.

The ideal candidate will have a current license to practice as a Nurse Practitioner in the state of Vermont with a minimum of (1) year Nurse Practitioner experience and a minimum of (1) year hospice or palliative care experience.
Must maintain proof of current CPR.
Excellent observation, problem solving, verbal and written communication.
