Seeking a Full-Time Permanent Nurse Practitioner to join a Hospice and Palliative Care Team!

Details:

Currently seeking a compassionate NP with a passion for end of life care. The NP will be responsible for providing comprehensive hospice and palliative care in consultation with the Hospice Medical Director. In collaboration with the interdisciplinary team, patient, and family, the NP will provide consultation, assessments, and evaluation of the plan of care in the hospital, and home settings. 

The ideal candidate for this role will have current license to practice in VT with at least 1 year of hospice or palliative care experience. Must maintain proof of CPR. Must have excellent problem solving, observation, verbal and written communication skills. Position offers competitive compensation and full benefits!
