This role involves delivering high-quality primary care to pediatric and/or adult patients in both ambulatory and inpatient settings, with a strong emphasis on prevention and health promotion. As part of our collaborative care team, you'll also provide clinical leadership, support the training of healthcare staff, and participate in community-focused health initiatives.

Qualifications:
- 2-3 years of direct patient care experience as a PROVIDER
- Current IL State License, Controlled Substance License, and DEA registration
- Bilingual English/Spanish, required</li>
