An active Texas LVN license with no discipline is required for this role.

Previous experience in primary care is required.

What will you do?
Utilize critical thinking abilities and problem-solving strategies in the management of chronic care
Maintain current knowledge of disease processes, resources, and treatment options available.
Achieve the set performance targets.
HIPAA-compliant use of computer access to facilitate patient care.
Use a care plan to guide patient interaction and direction.
Review individual care plans and make monthly phone calls to CCM-enrolled patients.
Manage the flow of information to and from the provider’s office.
Assists in the assessment of patient needs and health by taking patient history, medication listing, and assessing patient’s physical and mental status.
Complete all necessary documentation in the patient’s medical record in accordance with the practice’s charting policy.
Support Annual Wellness Visits by contacting patients to schedule appointments and collect data.
All other duties as assigned by manager

Skills

What are we looking for?
Primary care experience
Excellent communication skills, written and verbal
Experience using an EMR
Understanding of medical terminology including commonly prescribed medications.
Strong attention to detail
Excellent patient service and relationship skills
Bilingual, Spanish is a plus

What are the details?
This is a full-time position.
Hours are Monday through Friday, 8 a.m. to 5 p.m.
Compensation commensurate with experience.
Eligibility to excellent benefits package after 60 days.
