Description
Working with an interdisciplinary team of social workers, case assistants, attorney and pastoral counselor the RN would design protocols for health and wellness of older adults living in the community, including emergency response. Assess health needs and provide health advocacy, as needed. Develop working relationships with health and social service partners throughout NYC. Educate community partners and patients on evidence-based practices in public health and health literacy. Initiate projects with gatekeepers, physicians, and pharmacies to increase program reach. Research and refer patients to long-term public health, geriatric health, mental health and social services in NYC. Collaborate with JASA social workers, CHHA nurses, and involved family members to provide integrated geriatric care services. This is a full or part time position working between the hours of 9 to 5 pm Monday to Friday.
Skills
QUALIFICATIONS:Current license and registration to practice as a Registered Nurse in NYS.
Must be computer literate, including ability to learn and use program-specific software.
Experience with older adults preferred.
Willing and able to work a flexible schedule in the event of an emergency which may include nights and weekends.
Excellent interpersonal skills, including the ability to communicate effectively and concisely (both in writing and orally)
Strong attention to detail with the ability to solve problems, prioritize work, and work independently
Ability to travel to visit clients and other partners
