In this position you will ensure timely and efficient patient referrals and transition throughout the continuum
of care from diagnosis to treatment to post- treatment follow up with Oncology patients. Responsible for the management of resources
and services through interdisciplinary collaboration to achieve optimal,
contiguous patient outcomes for oncology patients.

The Adult Oncology
Nurse Navigator’s primary function is to ensure the patient
