Key Responsibilities

• Evaluate, plan, and implement comprehensive treatment programs for patients in the outpatient setting

• Educate patients and families regarding therapy plans and self-management strategies

• Collaborate with a multidisciplinary team to ensure high-quality care delivery

• Demonstrate clinical techniques and monitor progress, modifying plans as necessary

• Document evaluations, goals, treatments, and outcomes in accordance with policy

• Participate in performance improvement and continuing education initiatives
