Weekend Requirements: Yes; 4-week rotation

On Call Requirements: Yes; 2 weeks no call, 3rd week float, take call

Requirements:

• Active Arizona Medical License


• Clean malpractice background


• Board Certified


• CRNA Skills Checklist (to be uploaded with presentation)


• Availability for a minimum of 10 shifts per month (every week; Monday-Friday)


• Certification Requirements: ACLS, BLS, NBCRNA, PALS


• License Requirements: Arizona

Length of Need: Ongoing
Average Time to Credential: 60-90 days
Types of Cases: General surgery, OB/GYN, GI, Ortho, and occasionally Cath Lab (no hearts or neuro)
Majority Case Type: Ortho and General
Number of Operating Rooms: 2 main operating rooms
