• Affiliations go back 5 years


• Case logs last 24 months


• Must have NH license and DEA at the time of presentation


• Must have clean malpractice/background


• Must be Board Certified in Emergency Medicine


• Must send self-query NPDB and EPA with presentation


• Must submit COVID vaccination card


• ACLS needed if requesting sedation privileges

CERTIFICATION REQUIREMENTS:
