Physical Therapist - Home Health - (PT - HH)

Pay Rate: $2806.00 - $2891.00
Travel PT Home Health jobs in Southboro, MA are available for 13-week assignments with Monday through Friday 8-hour days. You must have an active Massachusetts license and home health experience. You will complete 6 to 7 patient visits per day, with visits scheduled in a tight radius covering Framingham, Natick, Hopkinton, and Westboro. This role uses the Homecare Homebase EMR system. Mileage is reimbursed at $0.72 per mile.
Your responsibilities include evaluating patient needs, developing and implementing individualized treatment plans, documenting progress in Homecare Homebase, and collaborating with healthcare teams to support patient mobility and independence
