We are currently seeking a skilled and compassionate Physical Therapists to join our team and provide in-home therapy services across Fairfield County, Litchfield County and New Haven County.

What You'll Do

As a Home Health Physical Therapist, you will:

Manage an assigned territory in either the North, Central or Southern area of Western Connecticut.
Perform comprehensive physical therapy evaluations and collaborate with physicians, nurses, patients, and families to develop individualized care plans.
Conduct OASIS assessments
Deliver goal-oriented, evidence-based therapy in the home setting to help patients regain function and independence.
Provide education and instruction to patients, families, and caregivers.
Identify and refer for additional services or next level of care as needed.

Compensation
The pay range for this position is $90,000 - $110,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.

Skills

What You Bring

Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
Connecticut PT license in good standing
2+ years of clinical experience (home health experience preferred)
Proficiency in OASIS documentation and EMR systems (tablet provided)
Strong understanding of therapy best practices in home-based settings
Excellent communication, time management, and problem-solving skills
Valid driver's license and reliable vehicle for travel throughout assigned territory in Fairfield County
