We are dedicated to doing this not only for our patients but for their families and loves ones as well.We are seeking a dedicated Palliative Care Nurse Practitioner contributes to the palliative care program and provides quality end of life care to patients and families. Clinical responsibilities include education and consultation for complex pain and symptom management, and advance care planning. Provides face to face palliative care clinical visits where the patient resides. Essential Duties & Functions: Schedules admissions and adheres to service standards. Serves patients in nursing homes and assisted living facilities, private residences, and in hospitals, in accordance with the Palliative Care Program. Obtains a comprehensive or problem specific history for patients of any age. Performs a complete or problem focused physical examination for patients of all ages. Orders and/or performs, evaluates, interprets, and integrates finding of diagnostic tests and procedures to formulate health management plan. Responsible for comprehensive assessment of medical, psychosocial, and spiritual needs of patients in conjunction with palliative physician and in keeping with interdisciplinary principles. Responsible for family meetings and other patient/family care planning, as well as collaboration with attending physicians, facility medical directors, and other physicians of patients served. Educates and counsels patients and families on palliative care and end of life issues. Educates and counsels Hospice, palliative, and facilities’ staff on palliative care and end of life issues. Implements and monitors patient care plan, coordinates with facilities and with Hospice on any referrals to Hospice.
Skills
Minimum two years experience as a Nurse Practitioner preferred. Experience in hospice and palliative care or pain management strongly preferred. Education: Master’s degree in Nursing Training & Licensure: IL Nurse Practitioner License, DEA License, Certification in specialty area, preferred, CPR certification, Proof of EOL education, hospice certification as ACHPN, required by second anniversary date, preferred; valid Illinois drivers license, auto liability insurance coverage and reliable transportation. Knowledge/Skills/Ability: Knowledge and clinical expertise to perform advanced care planning. Knowledge of Federal, State, and local rules and regulations.
