Are you looking for a new PMHNP position in Vermont? Looking to relocate to a beautiful ski resort location with endless outdoor activities? A community health center that is directly affiliated with a near by hospital is looking for a psychiatric mental health nurse practitioner to join their team! Option for 4 10 hour shifts or 5 8 hour shifts. Join a dynamic, collaborative team of Behavioral Health Professionals including Adult; Child Psychiatrists, Psychiatric Nurse Practitioners, Licensed Social Workers and Licensed Alcohol; Drug Counselors/Licensed Clinical Mental Health Counselors who together provide a plan of care for our patients. This position will involve coordinating plan of care with primary care providers to include psychiatric consults, completing diagnostic evaluations and ongoing medical management services. Nurse Practitioners also receive the support of RN Case Managers.
