Schedule: Monday through Sunday, 7:00a to 7:00p. 5 to 8 assessments per day. At least 1 weekday of availability is required. Weekend only availability is not accepted.

Position Overview

In home health and wellness assessments

Patient population: Geriatrics

Assessments typically last 45 to 75 minutes

Documentation completed on an iPad

Provider works independently with access to a provider hotline

Clinical Scope

H and P evaluations

Medication review

Vital signs

Finger stick testing

Urine sample collection

Fecal kit distribution

Required Qualifications

Active state NP license

National certification

NPI

CAQH (can be obtained during credentialing as applicable)

Unrestricted license

Auto insurance meeting client requirements

No DEA required

Credentialing and Onboarding

Credentialing is self paced and typically 30 to 45 days

Online training modules must be completed before scheduling
