Physical Therapist - Home Health - (PT - HH)

Travel PT Home Health jobs in Cranford, NJ are available for 13-week assignments with 8-hour day shifts and 5-6 visits per day. You must have home health experience and an active NJ license.
Your responsibilities include evaluating and treating patients in their homes, developing individualized care plans, documenting care and outcomes in Homecare Homebase EMR, and collaborating with physicians and interdisciplinary teams. You will cover Union County, NJ, with the most help needed in Monmouth County, and receive mileage reimbursement at $0.72 per mile
