Who we are seeking: 

• MD or DO graduate
• Family Medicine Residency trained
• New York State licensed or the ability to obtain one
• DEA certificate
• Board Eligible or Board Certified in Family Medicine
• Current Cardiopulmonary Resuscitation Certificate
