Description
Our client in California is seeking a dedicated Family Practice Nurse Practitioner for a 120-day assignment starting Jun 15, 2026. This role involves managing a patient volume of approximately 18 daily, maintaining patient refills, and providing comprehensive care across various domains. Shifts are scheduled from 8:00am – 5:00pm or 10:00am – 7:00pm. The Nurse Practitioner will address chronic disease management, perform annual wellness exams, provide employment physicals, manage basic mental health concerns, screen for HIV/STI, and offer specialized care for trans and non-binary patients, including hormone initiation, maintenance, and monitoring.
Responsibilities and Duties - Manage a patient volume of approximately 18 daily
- Maintain patient refills
- Provide comprehensive chronic disease management, including hypertension, Type 2 diabetes, and asthma
- Conduct annual wellness exams, employment physicals, and follow-up visits for chronic labs
- Manage basic anxiety and depression cases, including SSRI/SNRI titration, with referrals for specialized therapy as needed
- Perform screening for HIV/STI (PrEP & PEP)
- Initiate and maintain hormone therapy for trans or non-binary patients, prescribing and adjusting Testosterone (IM/SubQ/Gel) or Estrogen + Anti-androgens (Spironolactone)
- Monitor hormone therapy by checking total/free Testosterone, Estradiol, CBC for polycythemia, and BMP for potassium levels
